Adds `passthru.perVersion` (version -> single-version derivation) to each multi-version extension. The attrset already existed inline as the input to the buildEnv; this just surfaces it so the extension catalog can reference a single (extension, version) store path directly. No change to any buildEnv output. Part of PSQL-1530.
