docs: replace broken Slack badges with Discord#5529

The Slack invite badges on the server and client package READMEs point at dead hosts (slackin-socketio.now.sh and slack.socket.io). Point them at the active Discord invite instead, matching the badge used in the NestJS example. Fixes socketio#5229.
