docs: reposition README around primitives-first pivot#6

Lead with the validated tagline. Add a Why section that names the manual-grep-tracing failure mode and how code-reason closes it (deterministic, token-efficient, faster, trustworthy). Add a What you get section with dogfood numbers. Reorder Tools so navigation and data-flow primitives lead, with catalog-scan tools grouped as convenience. Add a positioning line vs. traditional SAST.
