  • 先看:docs/qa/platform-checklist/RUNNER.md 规则 2 的两条子项;.claude/skills/checklist-test/SKILL.md 的 Guardrails 与 §2。
  • 两分钟验证:pnpm check:pm-skill-ratchet && node scripts/check-platform-checklist.mjs,两者 exit 0。
  • 需要你做的:无,席位落地(Tier S,欠一次契约档复审)。

What changes

One rule, written in both places — docs/qa/platform-checklist/RUNNER.md rule 2 and .claude/skills/checklist-test/SKILL.md — and neither text claims precedence over the other.

Security findings (#27382). A security finding is an authentication or authorization hole, or any finding whose reproduction is an exploit recipe.

  • It gets no GitHub card at all. That includes the withheld-detail card, and it is not mentioned on the wave anchor or a tracking card either. Its reproduction appears nowhere on GitHub.
  • The run record keeps one row, area.item · clause · security finding — detail withheld, reported to PM. That row is a completed verdict, and the title counts it as a FAIL.
  • The runner reports the reproduction privately to the session that dispatched it, by session-to-session message.
  • That PM session dispatches the fix through pm-dispatch: an in-seat os-dev subagent at xhigh, with the reproduction only in the dispatch prompt and hardening-only wording in the PR, commit and changeset. A finding that needs a contract decision first goes to the maintainer.

Runners only test (#27382, widened to issues and PRs).

  • Every GitHub write and every issue or PR handling step of a run (extraction, dedupe, close-out) goes through the pm-dispatch write doors.
  • A runner never spawns os-dev and never drives a PR. The dispatching PM session owns every PR the run produces: review, contract review where it is owed, readiness and landing.
  • When a runner's own permission check refuses a write door, it stages the writes in a file, reports the path and hands off. The dispatching session reads them and posts them through the same doors in the staged order. A staged file never carries a security reproduction.

Runner effort (#27390), skill §2 only.

  • Effort by card shape, stated in every runner dispatch, never empty. A run card (Tier 1 pin batch, manual API/CLI or browser set, P0 smoke, UX walkthrough) is high. xhigh is only for the os-dev that fixes a security finding and for adversarial re-verification of a contested verdict.
  • Per backend:
    • An Agent subagent gets effort passed explicitly, and the skill line is what makes passing it lawful.
    • A cloud card (create_session) takes no effort. Its record carries no effort field either (measured, below), so the dispatch note says "not set" plus the dispatching session's own level. When that level is above the card's shape, the dispatcher follows the dispatch-runbook's 〈云卡五课〉 item 五 ② line, which the skill cites rather than restates, or opens a project thread instead and sets its effort at spawn.
  • The spend line: the wave anchor's dispatch note and each runner's final report carry each card's spend at report time, read from get_session external_metadata.usage.cost_usd.

Files.

  • docs/qa/platform-checklist/RUNNER.md: rule 2's carve-out is rewritten, and a new sub-bullet, "The runner only tests", is added. Two later mentions of the retired card shape now match it: the Run records paragraph and the Extraction obligation bullet.
  • .claude/skills/checklist-test/SKILL.md:
    • Guardrails: the security bullet is rewritten, and a "runner 只测试" bullet is added.
    • §4: the extraction line is rewritten, and the write-door paragraph now covers issue and PR handling and the refused door.
    • §2: three bullets cover effort, backends and spend, and the QA-run line names PR ownership.
  • .claude/agents/os-dev.md: two existing lines now carry the no-card dispatch. The dispatch prompt stands in for the card, there is no Fixes #, and the report is the final message only. No line is added (402 lines, ceiling 402).
  • scripts/pm/check-skill-line-ratchet.mjs: the checklist-test/SKILL.md ceiling goes from 246 to 259.

Why

The maintainer's words on 2026-10-29 in the dispatching session, verbatim and untranslated, all collected on #27377 in comment 6500750765 (the ruling of record):

pm-skill line ratchet raise. Ruling of record: #27377 comment 6500750765. Measured line delta: +13, .claude/skills/checklist-test/SKILL.md 246 → 259. The CEILINGS row moves by exactly that amount. No other ceiling moves, and os-dev.md stays at 402.

Measured before writing (#27390 item 2)

All readings were taken with get_session on 2026-10-29, between 15:20Z and 15:30Z.

What this shows:

  • A project thread runs at the level it was given at spawn, not at its parent's.
  • A create_session card's record carries no effort level at all, so nobody can read back the level it ran at. That is why the skill tells the dispatcher to record "not set" and the dispatching session's level, and does not claim an inherited level.
  • usage.cost_usd sits at external_metadata.usage.cost_usd, and a session with no finished turn has no usage block yet (this thread's own record at 15:18Z).

Verification

All at head 6b95c870, on a fresh worktree off main at 9c513147.

  • pnpm check:pm-skill-ratchet (self-test and gate): exit 0. checklist-test/SKILL.md is 259 lines against a ceiling of 259, every line is within 120 bytes, and the table-row pin is unchanged. os-dev.md is 402 against 402.

Acceptance notes

  • The file surface is widened by two RUNNER.md mentions outside rule 2. The Run records paragraph and the Extraction obligation bullet repeated the retired withheld-card shape, so left alone they would have contradicted rule 2. They are in the same file, and docs/qa/** is not governed. Declared to the seat.
  • Three checklist item strings still quote the retired detail withheld pending maintainer wording, as pointers to rule 2's carve-out: one in areas/access-security.json and two in areas/identity-auth.json. They defer to rule 2, which governs. Rewording them is an item revision (a revision bump and a history row), which is outside this card's surface. Left for the seat: either a follow-up or noted only.
  • pm-dispatch dispatch-runbook.md 〈云卡五课〉 item 五 ② says a session that cannot take effort inherits the seat's level. The readings above cannot confirm that for create_session cards, because their record has no effort field. The runbook is not edited here (pm-dispatch is outside this surface). The skill cites the line and records what can be read.
  • Tier. Touching .claude/agents/os-dev.md makes dispatch-gates --tier derive a MANDATORY contract-review tier for the surface. The two-line amendment there is one-line-class and mechanical. The build ran at the default tier, and the contract-tier review the seat arranges is the compensating control.
  • No PR assignee. os-dev.md says a PR carries none, and the maintainer reaches the PR through the seat's hand-off.

Human Minimum

  • 1. Schemas start as Zod; packages/spec holds no runtime logic. (packages/spec is not touched.)
  • 2. Every name I added uses the casing for its kind.
  • 3. No formatter ran; pnpm lint passes. (eslint on the one .mjs file changed: exit 0.)
  • 4. One issue, one branch off an up-to-date main, one pull request. (Two cards folded into one PR by the seat's dispatch order; one branch off main at 9c513147.)
  • 5. A change that publishes carries a changeset, and both declaration lines are filled in here and in the changeset. (Nothing publishes: skip-changeset. Both declaration lines are above.)
  • 6. packages/spec touched: its generated artifacts are regenerated and committed. (Not touched.)
  • 7. No hand edits to content/docs/references/, packages/*/CHANGELOG.md or content/docs/releases/.
  • 8. Title, body and comments are in English. (The 维护者速读 block and the quoted rulings are Chinese, as the repo's rules require.)
  • The Before Submitting list in CONTRIBUTING.md is done.

…vately to the PM session, runners only test, and runner dispatches state effort by card shape

RUNNER.md rule 2 and the checklist-test skill carry one rule in two places.
A security finding (an authentication or authorization hole, or any finding
whose reproduction is an exploit recipe) now gets no GitHub card at all; the
run record keeps one row, and the runner reports the reproduction privately
to the session that dispatched it. That PM session dispatches the fix through
pm-dispatch and owns every PR the run produces. Every write and every issue or
PR handling step of a run goes through the pm-dispatch doors; a refused door
stages the writes and hands them to the dispatching session.

The skill's section 2 now states runner effort by card shape (high for a run
card, xhigh only for a security-fix os-dev and adversarial re-verification),
how each backend takes it, and the spend line. os-dev.md gains the no-card
report path in two existing lines. The checklist-test ceiling rises 246 to 259
under the ruling of record that orders the text.

…ot the retired card wording

access-security.platform-owner-email-anchor, identity-auth.two-factor-enrollment-reveal
and identity-auth.two-factor-verify-to-activate pointed at RUNNER rule 2's carve-out in
the retired shape ("item + clause + detail withheld pending maintainer", and "existence
published"). Each now names what rule 2 says after this PR: no card, and one run-record
row `area.item · clause · security finding — detail withheld, reported to PM`. Revision
bumped with a history row on each; no clause, oracle or status changed.

A second commit, 2d15ca6c, was added at the Devx seat's request. Three checklist items pointed at rule 2's carve-out in the retired shape, and they now name the new row.

  • The strings changed are access-security.platform-owner-email-anchor negative[0], identity-auth.two-factor-enrollment-reveal negative[2] and acceptance[4].verify, and identity-auth.two-factor-verify-to-activate acceptance[2].verify. Each now says no card and names area.item · clause · security finding — detail withheld, reported to PM. Each item gets a revision bump and a history row. No clause, oracle or status changed.
  • This supersedes the body's second acceptance note ("Left for the seat"). The body itself is unchanged, because the relay's issue_patch door wants a signature line as the first line, and a PR body does not carry one.
  • The gates were re-run in full on 2d15ca6c. dispatch-gates --ran shows 43 derived and 43 run, every one exit 0. check-platform-checklist exits 0 (16 areas, 352 items). Changed lines: 144 (+96 / −48).

Served-tier: CONTRACT_REVIEW_TIER
Head-sha: 2d15ca6c49115d8129dccbd625ee221f74bb1db9
Local-runs: none
Scope: .claude/agents/os-dev.md, .claude/skills/checklist-test/SKILL.md + docs/qa/platform-checklist/RUNNER.md, scripts/pm/check-skill-line-ratchet.mjs, .claude/skills/pm-dispatch/references/dispatch-runbook.md, .claude/skills/pm-dispatch/references/execution-duties.md, .github/workflows/pr-body-guards.yml, scripts/check-commit-card-trailers.mjs, docs/qa/platform-checklist/areas/access-security.json, docs/qa/platform-checklist/areas/identity-auth.json

Inputs read: card #27382 (body + 5 comments, rulings 6493929972 and 6493958591, claim 6500623670, os-dev report 6501662628), card #27390 (body + 2 comments), PR #27642 (body, 6-file list, net diff against main, 2 comments), the 44 check-runs on the head. Hits files read whole in the committed tree; cited code read on the head where the PR touches it and on origin/main where it does not. The rest of the diff (RUNNER.md, the two area JSON files, the ratchet script) is the lane seat's ACCEPT at TIER_DEFAULT; it is read here only to judge the two hits files.

① Derived judgments

② Semver level

skip-changeset, Clause-②: no, Metadata-protocol: none — on the PR body and on both claims (6500623670, 6500651577). The diff touches no published package, no spec, no generated artifact, no skills/**; Check Changeset skipped. Nothing publishes, so no level is owed and skip-changeset matches the diff. Clause-②: no is right: no published accept set is narrowed; the only accept sets that change are agent-rule accept sets inside .claude/**, which clause ② does not grade. Both declaration lines are carried where os-dev.md says they must be (PR body line-start). Right.

③ Boundary flags

  • os-dev report open_questions: empty. Nothing to answer.
  • out_of_scope_findings 1 (three checklist strings still quoting the retired row): settled on this head by the second commit; the diff re-words access-security.platform-owner-email-anchor negative[0], identity-auth.two-factor-enrollment-reveal negative[2] and acceptance[4].verify, identity-auth.two-factor-verify-to-activate acceptance[2].verify, each with a history row. A tree grep for detail withheld pending maintainer on the head finds only history rows. Answered.
  • out_of_scope_findings 2 (runbook 〈云卡五课〉五之 ② inheritance unverified for create_session): the skill records exactly what the measurement supports (effort_level absent on create_session records, so "未指定" plus the dispatching session's own level) and cites the runbook line instead of restating it. The runbook line exists at dispatch-runbook.md line 124 under lesson 五. Lesson 五 is the desktop start_session reading, not create_session; what the skill borrows from it is the remedy ("或从卡档位的会话发起"), which is backend-neutral. Accepted, not escalated; the runbook is pm-dispatch's surface.
  • out_of_scope_findings 3 (file surface widened by two RUNNER.md paragraphs; os-dev.md makes the tier MANDATORY): the two RUNNER.md paragraphs would otherwise contradict rule 2 in the same file; docs/qa/** is ungoverned. The MANDATORY tier is served by this record. Answered.
  • Triage seam 6488310446 ("the fix dispatch also lands somewhere on GitHub"): closed by the ruling itself (in-seat subagent, no lane card) and by RUNNER rule 2 on the head: "not the run issue, not a card, not a comment, not a PR". Answered.
  • Card comment 6493929972's two asks (what a runner does when a door is refused; what the dispatching session may do with the staged writes): both are now written, but the hand-off as written does not reach a cloud runner's file — see ④ must-fix 1.
  • Human Minimum item 4 unchecked (two cards, one PR): the fold is the seat's dispatch order on both claims. Accepted.
  • Dev flag "No PR assignee": os-dev.md rule 2 says the same. Accepted.

④ Design judgment

.claude/skills/checklist-test/SKILL.md — judged against #27382 (body, 6493929972, 6493958591), #27390 (body), RUNNER.md rule 2 on the head, pm-dispatch's runbook and execution duties, and the write doors in scripts/pm/.

Holds:

Must-fix 1 — the refused-door hand-off no longer reaches a cloud runner's staged writes. On main, §4 reads "待发文本落文件、终报给出路径与正文,放行后照序补发": the final report carried the path AND the text. On the head it reads "待发文本落文件,路径交派发会话,由它读过照序经同一写入门补发": the path alone. The scenario this rule is accepted against is card comment 6493929972's wave-3 reading: W3-C1, W3-C2b and W3-UX were create_session cloud cards (#27390 body) whose own permission check refused issue-create.mjs and post-stamped.mjs. A file in a cloud runner's container is not readable by the dispatching session; a path there is not a hand-off, and "由它读过" cannot be executed. The dispatching session is left to re-contact the runner or mine its transcript, which the text neither says nor requires, while the rule's own closing clause ("写入未落地即未收口") keeps the run open. Main's wording held for both backends; the head's does not, and the same regression is written into RUNNER.md rule 2 ("reports the path and hands off to the dispatching session, which reads them"). Fix: the staged text itself travels with the hand-off (in the final report, or over the same session-to-session channel), both places, and a security reproduction stays excluded from it as the head already says. Line 140 is at 119 bytes and line 141 at 108, so the phrase fits by re-wrapping without a new line.

Should-fix 1 — §5's four-way disposition is declared exhaustive ("四选一是穷尽的 … ⛔ 永不把它留成一条无处置的红行") and its 产品缺陷 row says "抽取独立缺陷卡"; the security FAIL row fits no row and is forbidden a card. The two ⛔ lines collide at close-out, and the machine-grep close-out grammar (Extracted: #缺陷卡号 · area.item · 条款) has no shape for a row with no card. §4's pointer to the Guardrails resolves it for a careful reader, but the exhaustiveness claim is now false; one clause naming the security row's disposition (closed by its own row; no Extracted: line) belongs in §5.

Should-fix 2 — the old fallback "runner 无派发通道时由派它的会话接手" was removed and nothing replaces it for a runner with no session-to-session channel. The channel is measured for the wave-3 shape (W3-C3b, 6493958591), and a subagent's final report reaches its dispatcher by construction, so this is narrow; but pm-dispatch's runbook lesson 五 ⑥ records a cloud shape that can neither receive a poke nor send a message. One clause saying what the runner does then (holds the reproduction in-session, reports blocked without it) closes the gap.

Should-fix 3 — §2's "pm-dispatch 管写入门(§4)、安全修复派卡与 PR 持有(Guardrails)": 派卡 reads as dispatching a card, and the Guardrails it points at say no card is dispatched. "安全修复派发" or "派修复" says what the rule does.

.claude/agents/os-dev.md — judged against #27382 item 3's sub-bullet (state how os-dev reports with no issue: no Fixes # line, report to the dispatching session), the claim's surface ("only for the no-issue report path"), the PR-body guards and the commit-trailer gate.

Holds:

  • Line 15 "无卡派发时派发令即卡" and line 16 "无卡则无 Fixes #,仅交终报" answer the card's two asks in two existing lines; no line added, 402/402, both lines inside 120 bytes.
  • The gates agree with the rule: pr-body-guards.yml guards nothing when no closing keyword is declared; commit trailers already carry no card relation; the no-card hardening PR is landable through the PM session that owns it (RUNNER rule 2, SKILL Guardrails).
  • check:pm-skill-id-lint green: no issue number entered the file.
  • Report delivery: "仅交终报" overrides the two-delivery rule on line 17 for the no-card case, and the terminal JSON reaches the dispatching session, which is the reader the card names.

Should-fix 4 — the rest of the file stays keyed on an issue number the no-card path does not have: rule 1's worktree and branch (claude/issue-N-slug), the scratchpad subdirectory, rule 3's three-write budget (the report comment is one of the three), the terminal JSON's issue field and branch value, and the frontmatter description ("Developer agent for exactly ONE GitHub issue … never for open-ended … work"), which is what the Agent tool reads when it selects this agent. "派发令即卡" carries the intent, and line 383 ("报告模板是工具,不是真相") lets the dev state a field that does not fit, but the dispatch order is nowhere told to name the branch and the issue value. One clause — the no-card dispatch order names the branch, issue is null — and one word in the description would close it without a new line; the file has no headroom, so it is an in-place edit.

Verdict basis: must-fix 1 is a regression from main in a hits file, in the exact scenario the clause was written for; it fails the record.

…itself, and a security row closes on its own

Contract-review findings on the previous head.

- A runner whose own permission check refuses a write door now hands the
dispatching session the path and the staged text, in its final report or by
session-to-session message. The dispatching session cannot read a file in a
cloud runner's container, so a path alone was not a hand-off. This is
written in both places: RUNNER rule 2 and the skill's section 4.
- A runner with no session-to-session channel keeps the reproduction in its
own session and reports the hand-off as blocked, without the reproduction.
This is also written in both places.
- Section 5's four-way disposition now says it is exhaustive except for a
security finding, whose row is its own disposition: no card and no
  Extracted line.
- Section 2 says the PM dispatches a security fix, not a fix card.
- os-dev: a no-card dispatch order names the branch, the report's issue field
is null, and the description admits a no-card order. Line counts are
  unchanged: 259 against 259 and 402 against 402.

This answers contract review 6504269255. All five findings are fixed in 4e5517ca, the new head, and no line was added (checklist-test/SKILL.md is 259 against 259, os-dev.md 402 against 402).

  • Must-fix 1, the refused-door hand-off. The runner now hands the dispatching session the path and the staged text itself, in its final report or by session-to-session message. RUNNER rule 2 also says why: the dispatching session cannot read a file in a cloud runner's container. Both places say it: RUNNER rule 2 ("hands off to the dispatching session with the path and the staged text itself") and SKILL §4 ("路径与正文随终报或会话间消息交派发会话"). The staged text still never carries a security reproduction. In §4 the paragraph was re-wrapped in place to fit, with no new line.
  • Should-fix 1, §5's exhaustive claim. It now opens "除安全发现外,四选一是穷尽的", followed by "安全发现那一行本身即处置:⛔ 不立卡、不写 Extracted: 行,修复由派发它的 PM 会话派". The paragraph reuses its old orphan line.
  • Should-fix 2, a runner with no channel. It keeps the reproduction in its own session and reports the hand-off as blocked, without the reproduction. This is in both places: RUNNER rule 2's carve-out, and the Guardrails security bullet ("无此通道则留本会话,终报报 blocked"). That bullet was compressed in place: "⛔ 不立任何卡" replaces the longer wording, and the record row moves to its end.
  • Should-fix 3. In §2, "安全修复派卡" now reads "安全修复派发".
  • Should-fix 4, os-dev. Line 15 now says "无卡时派发令即卡并点名分支". The report template's issue reads <n | null — null only on a no-card dispatch>, and branch admits "the branch a no-card dispatch order names". The description now says "exactly ONE GitHub issue (or one no-card order)" and "a single fully-specified issue or order".

The gates were re-run in full on 4e5517ca. dispatch-gates --ran shows 43 derived and 43 run, every one exit 0. check:pm-skill-ratchet and check-platform-checklist both exit 0, and check:commit-card-trailers exits 0 over the three commits. Changed lines: 162 (+107 / −55). The head needs a fresh same-head contract review.

Verdict: ACCEPT at head 4e5517ca4d5a7f9256f9f46ef698cc8872df6f8c. Governed Tier S: check-governed-merges --pr 27642 puts 2 of 6 paths on the register, all under Tier S. Size 162 lines. It lands once a same-head contract PASS is on record: the FAIL record 6504269255 read 2d15ca6c, and 4e5517ca is the commit that lands its findings, so it owes the same reviewer's delta review. Landing also needs the maintainer's landing word for this term. needs:contract-review goes on with this ACCEPT.

Contract review

Served-tier: CONTRACT_REVIEW_TIER
Head-sha: 4e5517ca4d5a7f9256f9f46ef698cc8872df6f8c
Local-runs: none
Scope: .claude/agents/os-dev.md, .claude/skills/checklist-test/SKILL.md + docs/qa/platform-checklist/RUNNER.md, scripts/pm/check-skill-line-ratchet.mjs, .claude/skills/pm-dispatch/references/dispatch-runbook.md, .claude/skills/pm-dispatch/references/execution-duties.md, .claude/skills/pm-dispatch/SKILL.md, scripts/pm/check-half-states.mjs, scripts/pm/check-handover.mjs

Delta review on record 6504269255 (FAIL, head 2d15ca6c, this seat). Inputs read: card #27382 (body + the same 5 comments the previous record read; nothing new since), card #27390 (body + 2 comments), PR #27642 (body, 6-file list, net diff against main from the files endpoint, the 7 comments including the dev's answer 6504848066 and the seat's ACCEPT 6505199183), the 49 check-runs on the head. What moved: exactly one commit, 4e5517ca on parent 2d15ca6c, no merge and no main carry-over (merge-base with origin/main unchanged at 85663a57); it touches 3 files, +26 / -22: os-dev.md (3 hunks), SKILL.md (4 hunks), RUNNER.md (2 hunks). The net diff against main is still the same 6 files; the delta hunks in the two area JSON files and the ratchet script are nil. Each hunk of the move was read against the live tree on the head and compared with the finding it claims to land. Sections and findings the move does not touch stand as judged in record 6504269255, named below.

① Derived judgments

  • Face and tier: unchanged from record 6504269255 — both hits are governed register rows (.claude/**, Tier S), so the review owed is CONTRACT_REVIEW_TIER on the rules face; the seat's ACCEPT reads 2 of 6 paths on the register, all Tier S — right.
  • Nothing publishes; Check Changeset concluded skipped on the head, consistent with skip-changeset — right, as in 6504269255.
  • Public surface: none; SKILL.md stays internal: true, os-dev.md is a .claude/agents role — right, as in 6504269255.
  • Accept-set changes the move itself implies, each a dispatcher-side or runner-side rule with no runtime consumer: (a) a refused write door's hand-off is now accepted only when it carries the staged text itself (path plus text, in the final report or by session-to-session message), both in SKILL §4 and RUNNER rule 2 — right against card comment 6493929972's two asks and the wave-3 cloud-card scenario; (b) a runner with no session-to-session channel is accepted as blocked with the reproduction kept in its own session — right against ruling 6493958591 item 2 (the reproduction appears nowhere on GitHub) and the runbook's lesson 五 ⑥ shape; (c) a fail row of the security kind is accepted as closed by its own row with no Extracted: line — right against qa(checklist-test, RUNNER rule 2): a security finding met in a QA run gets no GitHub card at all — only a pm-dispatch fix dispatch — and every issue write of a run goes through pm-dispatch (maintainer, 2026-10-29) #27382 item 2 and the §5 close-out grammar; (d) os-dev's report now admits issue null and a dispatch-order-named branch on a no-card order — right against qa(checklist-test, RUNNER rule 2): a security finding met in a QA run gets no GitHub card at all — only a pm-dispatch fix dispatch — and every issue write of a run goes through pm-dispatch (maintainer, 2026-10-29) #27382 item 3's sub-bullet. No accept set outside the previous record's five findings moves.
  • Gate side, read as one with the rule: line counts on the head measured at 259 / 259 (checklist-test/SKILL.md, main 246, delta +13 unchanged) and 402 / 402 (os-dev.md); the CEILINGS row at scripts/pm/check-skill-line-ratchet.mjs line 1567 is untouched by the move and still cites ruling 6500750765. Every line the move wrote is within the 120-byte cap (SKILL lines 137 to 141 at 117 / 120 / 116 / 118 / 87 bytes, 187 to 189 at 119 / 119 / 43, 248 to 252 at 115 / 118 / 120 / 107 / 120; os-dev lines 4, 7, 15, 16, 362, 364 at 101 / 83 / 117 / 120 / 58 / 82). The lines over 120 bytes in both files are pre-existing table rows and fenced code, the structurally exempt classes; Lint · PM dispatch-gates concluded success, so the ratchet agrees — right.
  • The os-dev report template's issue field now reads n or null. Every GitHub-side parser of os-dev-report (check-handover.mjs reportedStatus, check-half-states.mjs H52 open_questions, record-recognisers.mjs) reads status and open_questions, not issue, and a no-card report is never posted to GitHub (os-dev line 16, "仅交终报"), so no parser sees the null; pm-dispatch SKILL.md line 280 says the report shape lives in os-dev.md alone, so there is no second copy to drift — right.
  • Check-runs on 4e5517ca: 49 runs, 28 success, 21 skipped, 0 failure, 0 in_progress or queued. The duplicated Auto Label and Check PR Size entries at 18:36Z are skipped re-triggers after their 18:19Z successes, not failures. Governed Surface Queue Guard, PR-body guards, Lint · PM dispatch-gates, Lint · repo & tooling gates, Lint · docs & code guards all success — the derived gate families the two hits files reach are green.
  • The rest of the diff (RUNNER.md, two area JSON files, the ratchet script) is the lane seat's ACCEPT at TIER_DEFAULT (6505199183); RUNNER.md is read here only because rule 2 is the twin of the skill text.

② Semver level

Stands as judged in record 6504269255: skip-changeset, Clause-②: no, Metadata-protocol: none on the PR body and both claims; the move adds no published path, no skills/**, no spec, no generated artifact; Check Changeset skipped on the head. Right.

③ Boundary flags

④ Design judgment

.claude/skills/checklist-test/SKILL.md — the four delta hunks, judged against #27382 (body, 6493929972, 6493958591), #27390 (body), RUNNER.md rule 2 on the head, pm-dispatch's runbook lesson 五 and execution-duties line 118, and the §5 close-out grammar on the head.

Must-fix 1 of 6504269255 (refused-door hand-off carried a path only) — landed. §4 lines 139 to 141 now read: 写入门被本会话权限拦下 ⛔ 不绕行:待发文本落盘,路径与正文随终报或会话间消息交派发会话,由它照序经原写入门补发;其中 ⛔ 无安全复现 —— 写入未落地即未收口. The staged text travels with the hand-off, by either channel a runner has (a subagent's final report reaches its dispatcher by construction; a cloud card's final report is read by get_session, which §2 already uses), the dispatching session re-posts through the same doors in the staged order, the security reproduction stays out of the staged text, and the closing clause still keeps the run open until the writes land. The twin in RUNNER rule 2 (lines 103 to 108) says the same and adds the reason (the dispatching session cannot read a file in a cloud runner's container). Both backends are covered again, as main's wording was; the regression is closed. The paragraph was re-wrapped in place (lines 137 to 141, longest 120 bytes), no line added.

Should-fix 1 (§5 exhaustiveness claim collided with the no-card rule) — landed. Lines 187 to 189: 除安全发现外,四选一是穷尽的 … 安全发现那一行本身即处置:⛔ 不立卡、不写 Extracted: 行,修复由派发它的 PM 会话派(Guardrails). The exhaustiveness claim is now true, the security row has a named disposition, and the machine-grep close-out grammar (lines 201 to 206) is not asked to carry a card number it does not have. The old orphan line (处 / 置 split) is absorbed, so the paragraph stays at three lines.

Should-fix 2 (runner with no session-to-session channel) — landed in both places. Guardrails line 249 to 250: 复现经会话间消息私报给派发它的 PM 会话(无此通道则留本会话,终报报 blocked); RUNNER rule 2 lines 91 to 93: keeps it in its own session and reports the hand-off as blocked, without the reproduction. This is the runbook lesson 五 ⑥ shape (no poke in, no message out; only PR and report comments go back), and the text keeps the reproduction off GitHub in that shape too, which is the ruling's hard line. The bullet was compressed in place (⛔ 不立任何卡 replaces the longer withheld-card sentence; RUNNER keeps the explicit "no withheld-detail card", and 不立任何卡 subsumes it) and the record row moved to the bullet's end; every clause of ruling 6493958591 item 2 and 3 is still present.

Should-fix 3 (§2 安全修复派卡) — landed: line 114 reads 安全修复派发与 PR 持有(Guardrails).

Everything else in the file stands as judged in 6504269255: the Guardrails "runner 只测试" bullet, the §4 extraction line, §2 effort by card shape, per-backend effort and the spend line, the one-rule-in-two-places clause.

Should-fix 5 (new, in the §5 region the move touched; not a FAIL). §5's three re-test triggers (lines 226 to 232) re-verify a fixed item through trigger 2, "QA 抽取的修复卡落地 ⇒ 接受该卡的座位把受影响的项追加到波次锚卡的重测清单". On main the security finding had a withheld card, so its fix landing fired this trigger. On the head a security hardening has no card, and the Guardrails forbid naming the item on the anchor or a tracking card, so trigger 2 cannot fire and the item is re-verified only by a release-candidate Tier 1 rerun (trigger 1, which does not reach a Tier 2 item) or the maintainer's word (trigger 3). The disclosure rule is kept, which is the ruling's priority, and the hardening PR carries its own tests, so this is narrow; but one clause on trigger 2 — the PM session that landed a security hardening re-runs the item itself, naming it on no card — would close the gap. The file has no headroom, so it is an in-place edit or a ratchet raise under the same ruling.

.claude/agents/os-dev.md — the three delta hunks, judged against #27382 item 3's sub-bullet (how os-dev reports with no issue), the claim's surface ("only for the no-issue report path"), and the GitHub-side readers of the report.

Should-fix 4 of 6504269255 (the rest of the file keyed on an issue number) — landed within the surface. Line 15: 无卡时派发令即卡并点名分支 — the dispatch order names the branch, so rule 1's claude/issue-N-slug has a stated substitute; the report template (lines 362 and 364) admits issue null, only on a no-card dispatch, and a branch the no-card order names; the frontmatter description (lines 4 and 7) now says exactly ONE GitHub issue (or one no-card order) and a single fully-specified issue or order, which is what the Agent tool reads when it selects this agent. 402 / 402, the edited lines at 101, 83, 117, 120, 58 and 82 bytes. The worktree directory name in rule 1 (-issue-N) is not re-keyed; with the branch named by the order and line 383's "报告模板是工具,不是真相", the dev has what it needs, and the claim's surface ("only for the no-issue report path") is not breached by the edits made. check:pm-skill-id-lint green on the head: no issue number entered the file.

The gates still agree with the rule, as judged in 6504269255: pr-body-guards.yml guards nothing when no closing keyword is declared, check-commit-card-trailers.mjs forbids card relations in commits, and the GitHub-side report parsers never see a no-card report.

Breaks nothing judged before. The delta hunks are confined to the five findings; the Guardrails bullet, §4 paragraph and §5 paragraph keep every clause the previous record listed as holding; RUNNER rule 2 still cites the 2026-08-18, 2026-10-27 and 2026-10-29 rulings side by side and still ends with "one rule written in both places, not a precedence claim by either"; the RUNNER Run-records paragraph (line 784) and Extraction-obligation bullet (lines 880 to 882) are unchanged and consistent with the rule.

Verdict basis: the one must-fix of record 6504269255 is landed in both places and in the scenario it was written for; the move lands the four should-fixes and adds nothing outside them; the check-runs on the head are 28 success, 21 skipped, none failing or pending. Should-fix 5 is new and narrow and does not fail the record.
