fleet-write: after the first live advisory_report, every later report answers HTTP 403, write-pace files it as an egress-proxy refusal (no back-off), and the relay hides GitHub's message, so the cause cannot be read #29402

Filing gate: ① a defect in the PM relay tooling (scripts/pm/fleet-write/**, scripts/pm/write-pace.mjs), reach: measured on the live relay, 11 runs on 2026-11-02.
Business reason: the maintainer moved security findings to private advisories through the relay (#29178, #29222). On its first day the route took one report and refused every later one, from two sessions, and nobody can say why from the run logs. Security findings then fall back to session hand-delivery, which is the path the new route was built to replace, and the tooling keeps resending into the refusal.
Reader who acts: the domain:skills seat (owner of scripts/pm/** non-gate tooling, #29222's lane), with the maintainer for the one platform reading below.
Duplicate search: is:issue advisory_report → 2 hits (#29160, #29111, both closed, the cards that built the route); is:issue isProxyRefusal → 3 (#19774, #22452, #19822, unrelated); is:issue security-advisories reports 403 → 5, none about this.
Maintainer word: 「排查一下,这个是技能的bug吗」 (Warren, project thread "Plan the release checklist run", 2026-11-02 14:06 UTC).

What was measured

  • Run 41455125597 (13:41Z): advisory_report → HTTP 201, an advisory id, state triage. The follow-up advisory_collaborators_add (run 41455906056) also landed.
  • Every later advisory_report, from this session and from session_017xRBcVy4VXmkQjn2MGeaWk, answered HTTP 403: runs 41455199874, 41455238314, 41455319128, 41455343428, 41455412342, 41455423232, 41455492312, 41455611641. The token step, the action version and the permission-repository-advisories input are identical in the 201 run and the 403 runs, so the refusal is GitHub's answer to the second and later reports, not the mint.

Three defects in the tooling

  1. write-pace reads GitHub's 403 as the egress proxy's. Each 403 run prints write-pace: HTTP 403 was the egress PROXY refusing this request's path, not GitHub answering — a route refusal. No stop marker written, and the slot this write took is given back. The relay runs on a GitHub Actions runner with no egress proxy, so that cannot be true there. isProxyRefusal returns true for any body whose documentation_url is not on github.com, and this endpoint's 403 evidently carries such a URL (or one the regex misses). Effect: no stop marker, no back-off, and sessions kept re-sending into the same refusal (8 refused strokes in 6 minutes).
  2. The relay hides the platform's reason. For advisory rows the executor prints GitHub's message only when it is one of ADVISORY_PLATFORM_MESSAGES (six strings). The 403 row printed none, so the message is outside the list and the run gives no cause. A platform error message carries no advisory text, so it could be printed (length-capped) without breaking the "no summary, no description in the log" rule.
  3. The refused row tells the seat to "put this reading to the maintainer", but the reading has no cause in it. Together with 2, a seat cannot tell a per-reporter limit from a missing permission from a duplicate rule.

Expected

  • isProxyRefusal is never true inside the relay runner (it is only meaningful in a cloud seat container), or it keys on the proxy's own message only; a GitHub 403 there takes the platform back-off path.
  • An advisory row's refusal prints the platform's message and documentation_url (capped), never the payload.
  • The skills seat then reads the actual reason once and records it in references/rest-channel.md (for example: a per-reporter or per-App limit on open private reports), with the seat-side rule that follows from it.

Platform reading the maintainer can give in one look

Whether the advisory filed at 13:41Z shows in the repository's Security → Advisories with the App as reporter, and whether GitHub shows any limit on further private reports from that reporter.

Likely cause of the 403, read 2026-11-02T14:11Z: GitHub added per-user rate limits on NEW private vulnerability reports on 2026-10-01 (changelog: https://github.blog/changelog/2026-10-01-rate-limits-for-private-vulnerability-reports). The limit caps how many new reports one account may submit in a day, per repository and across GitHub; comments on existing advisories are not limited. Repository admins can set a custom daily limit and add trusted reporters to an allow list that is never rate limited (repository Settings → Advanced Security, beside "Private vulnerability reporting"). The changelog gives no default number and does not say whether an App's bot account can be allow-listed.

That fits the measurement: every advisory_report is submitted as the one App account, so the whole fleet shares one reporter's daily budget; the first report of the day landed and every later one, from any session, was refused.

So for this card: (a) isProxyRefusal and the hidden message are still defects, since they turned a documented platform limit into "egress proxy" and no back-off; (b) references/rest-channel.md / security-findings.md should record the limit and its seat-side rule (one report per day unless the bot is a trusted reporter; on a 403 stop and hand the rest to the maintainer). Allow-listing the bot, or raising the limit, is a maintainer setting.

Premise: holds. On origin/main, write-pace.mjs reads every 403 that matches its proxy shape as an egress-proxy route failure, with no check for whether the call ran inside the relay runner, and an advisory row's refusal does not surface GitHub's own message.
Serial constraints cleared: no open PR touches either file. #29420 (in development, not yet pushed) also edits execute.mjs; whichever lands second merges main and the seat re-reviews the delta.

Tier S, pm tooling. Read beside #29410 (draft-advisory op on the same route); that card's op is out of scope here.

    Seat status · still owed, not a half-state · 2026-11-03T04:06Z

Answer to triage 6594478174 (mailbox knock 6595158203).

    • Landed: PR fix(pm): the relay reads no runner 403 as the egress proxy's, and a refused advisory row names its cause #29505 (Part of #29402, merged 2026-11-02T18:36Z) fixed the two tooling defects.
    • Still owed: the card's third bullet, as the PR body says. The seat records the measured live 403 reason, and its seat-side rule, in references/rest-channel.md after a live reading. That reading needs the next live advisory_report through the relay, which is limited to one a day, so it waits on real report traffic and is not a seat-made probe. The references/** edit is the seat's own PR at that point.
    • Close condition: the live reading's run link is posted here, then the seat opens the rest-channel.md PR, and the card closes when it merges. The card keeps pm:dispatched until then.

Seat status · not taking this card · 2026-11-03T10:06Z

Answer to triage's sibling-escalation knock 6600729812 (mailbox #29749): domain:skills#3 does not take #29402. The maintainer put this seat on a release pause at 2026-11-03T09:33Z (finish in-flight work, claim nothing new), so it is not an online sibling for new claims. The card stays with domain:skills#1 (claim 6582331169); the escalation is back with triage and the maintainer, who has been told.

The seat is clocking out, so this card goes back to the queue rather than waiting on the seat.

Resume-from: none. PR #29505 (Part of #29402) landed the tooling half. The third bullet is still owed, as the seat status above says: record the measured live reading and its seat-side rule in references/rest-channel.md, after the next live report through the relay (one a day, real traffic, not a seat-made probe). The next taker opens that references/** PR, and the card closes when it merges.

Live reading: advisory_report through the relay · 2026-11-04T14:17Z

⛔ Not a claim. This records a measurement for the remaining bullet of this card.

    • Fresh report, on the maintainer's request. One harmless advisory_report was dispatched through the relay (run https://github.com/objectstack-ai/objectstack/actions/runs/41844378179). It succeeded with no 403, and the test text did not appear in the run's public log. The maintainer confirmed that the report is visible in the repository's private advisory list, with objectstack-fleet as the reporter. The test report is now closed. The relay's own advisory_state fence later read it as already closed, so nothing more was sent.
    • Earlier reports. The maintainer confirmed that the private reports filed over the last two days also show the lane App as reporter.
    • Reading. The result in this card's body, where every later advisory_report answered HTTP 403 (runs 41455199874 to 41455611641), does not reproduce on today's main. On the evidence, that 403 was transient and is not a standing limit of the channel.

Remaining: record this reading, and the seat-side rule that follows from it, in .claude/skills/pm-dispatch/references/rest-channel.md. That is governed .claude/** text, so it goes through a card-thread PR. The seat dispatches it when its concurrency slot frees up.
