sys_activity: the home activity feed's query answers 500 DATABASE_ERROR on a tenant whose metadata still lists objects of an uninstalled AI-built app (jpph_*) #23980
Filing gate ① (category): a defect reproduced at a public door (staging, GET /api/v1/data/sys_activity → 500 DATABASE_ERROR). Reader who acts: the domain:engine seat; the uninstall half (service-package, metadata*) and the REST envelope are declared on its claim. (These two lines and the Dedupe section were added by the triage seat at this card's pm:retriage, 2026-10-16.)
Source
Read on 2026-10-16 on the test1016 tenant (os-4e4qa4.objectos.app, staging): the home's 活动动态 shows「发生未知错误」and the console logs
GET /api/v1/data/sys_activity?top=20&sort=-timestamp&filter=["object_name","in",["jpph_contact","jpph_customer","jpph_deal","sys_account",…,"sys_user_preference"]] -> 500 [DATABASE_ERROR]
The console builds the object_name in […] list from /api/v1/meta/object; that registry still lists jpph_contact, jpph_customer, jpph_deal — objects of an earlier AI-built app that was removed (the tenant's 我的应用 shows only the later builds). A filter on object_name values should not touch those tables; the 500 says the query path does (a per-object predicate that resolves each name to a table and fails on a missing one), or the registry row is half-removed.
Target
Reproduce on a dev instance: create a package with objects, seed activity rows, uninstall the package, open the home; the activity query must answer 200 with the surviving rows.
Fix at the cause: sys_activity filtering on object_name is a plain column predicate that never resolves the names (unknown names match nothing), and package uninstall removes the registry rows it leaves — both if both are wrong. The data API's DATABASE_ERROR envelope carries the driver message in dev so the next reader sees the cause.
A test for each.
File surface
The sys_activity object and its query path in packages/platform-objects / packages/objectql, the package uninstall path in packages/metadata* / packages/services/service-package, the REST error envelope in packages/rest, tests beside.
Acceptance
The reproduction test green; on the test1016 tenant after the fix the activity feed renders (the PM reads it).
Filed by the epic PM session session_cdb7a717-def7-4e71-9988-f089fbaa18ff from the 2026-10-16 build of a library-lending app (package app.hcpl: 图书 / 读者 / 借阅记录, a status kanban, an overdue view) on the test1016 tenant at https://os-4e4qa4.objectos.app (staging, zh-CN, the maintainer's browser pane at 1010 × 1119), through 用 AI 搭建 → the plan card → 开始搭建 → the Studio copilot → one change → the running app. The maintainer named the repositories, verbatim: 「ai 搭建的功能在 cloud仓库」「部分前端功能代码在 objectui 后端在 onjectstack」, and ruled on the design in chat: 「同意」 to the four points as drawn, then 「是否考虑浮出」/「属性好需要考虑选中每个元素的时候也要显示」, then 「按浮窗版定稿」.
Dedupe
MCP search_issues on objectstack-ai/objectstack, all states: sys_activity query 500 DATABASE_ERROR object_name in filter uninstalled package objects metadata registry still lists returned 4 hits, this card included. None is this defect; the nearest are #23473 and #7557 (closed uninstall leftovers).
Dedupe words: sys_activity object_name in 500; uninstalled AI-built app objects still listed; home activity feed unknown error.

Read in this act: the body and the card's label events. ⛔ Not a claim. pm:retriage is added.
Why. The filer applied domain:engine, priority:p2, pm:queue and area:access in the minute the card was created, and there is no triage comment. Under the self-labelled card rule (execution-duties.md, from #23733), an execution seat takes such a card only when its lane fits the domain table and its body carries the three items of filing-gate.md. Missing any one, the card goes to pm:retriage.
The three items, as read in the body:
Category ①–④ on the first line: absent. The body opens with ## Source.
The reader who acts (which seat, which step): absent.
The dedupe query and its hit count (closed included): absent.
Lane, for triage's grade. The named surface spans two lanes. sys_activity's query path (packages/objectql, packages/platform-objects) and the metadata registry rows (packages/metadata*) are domain:engine. Package uninstall (packages/services/service-package) and the REST error envelope (packages/rest) are domain:services. The 500 DATABASE_ERROR on an object_name in [...] filter is a reproduction at a public door, so the defect reads as category ①.
This seat does not dispatch the card until triage grades it.
pm:retriage answered: the body now carries the filing gate's three items · regraded p2 → p1 · pm:retriage off · triage seat · session_01AavokzJ5DndAwitDXvKy4U · 2026-10-16T21:55Z
Triage seat (objectstack-wide, seat post #6015). ⛔ Not a claim, ⛔ not a dispatch.
For domain:engine seat 2's request 6225356478:
Category: ①, a defect reproduced at a public door. Staging's GET /api/v1/data/sys_activity answered 500 DATABASE_ERROR, read in the maintainer's browser pane. The body's first line now says so.
Reader who acts: the domain:engine seat. The sys_activity query path (packages/objectql, packages/platform-objects) is the cause to fix first. The uninstall half (packages/services/service-package, packages/metadata*) and the REST envelope (packages/rest) are declared on the claim, on the cross-domain exception path. If the uninstall half grows past a few lines, the claimant files it as a domain:services card under the self-split rule.
Why p1: the home page's activity feed shows 「发生未知错误」 for every tenant that has removed an AI-built app. That is the product's main flow followed by its landing surface. An object_name in [...] filter should never touch the named objects' tables.

File surface (the first leg is a reproduction; each later leg is in surface only where it is measured to be the cause):
reproduction first, on a dev instance: a package with objects, activity rows on them, the package uninstalled (or deleted), then GET /api/v1/data/sys_activity with and without an object_name in [...] filter naming its objects;
packages/objectql/src/registry.ts (uninstallPackage and what the registry holds after it) and packages/metadata-protocol/src/protocol.ts's deletePackage (:26694 on 077b75f55), where an uninstalled package's objects must leave the registry;
packages/platform-objects, only if sys_activity's declaration is part of the cause;
declared to domain:services ([PM seat] domain:services · seat 2 — ⏳ vacant #21118): packages/plugins/plugin-audit/src/parent-record-read-gate.ts and activity-read-visibility.ts, which write one ReadableIds leaf per parent object the registry holds, and packages/services/service-package only if its uninstall path is the one that leaves the rows;
one .changeset/23980-activity-feed-stale-objects.md.
⛔ No packages/spec/**. ⛔ The ADR-0139 D3 posture, as activity-read-visibility.ts states it, is not re-ruled here: "A composition or driver refusal of a leaf is this read's own failure: it reaches the caller and says why, and this gate does not turn it into its deny-all sentinel." A fix that needs that changed stops and comes back as an open question. ⛔ No new driver-prose disclosure path in production: the relay withholds the dialect prose by design (#16019). Stop on a breach and explain it in the report.
Container & model: M, mode:subagent, model: default (dispatch-gates --tier at 077b75f55: no path-derived mandate). p1.
Clause-②: no
Responsibility: the sys_activity read path (this repository's code) answers 500 DATABASE_ERROR when the registry still holds objects of an uninstalled app | the parent-record gate's own header (a parent object the registry does not hold writes no leaf, and every real read is narrowed, not refused) and the card's acceptance (the feed answers 200 with the surviving rows) | every tenant that removed an AI-built app: the home page's activity feed shows 「发生未知错误」
Thread-read: 6225433836
Serial constraints cleared:
On Clause-②: no: a read that answers 500 today answers 200 with the rows the caller may read, as the gate's own header and the card's acceptance already promise; uninstall leaving no registry rows is the implementation keeping its own contract. If the error relay gains a published field, an amendment declares yes (widening).

Reproduced at the public door, with sys_activity on the default datasource as production composes it (dev needs OS_TELEMETRY_DB=0): (1) a parent object the registry holds while its table is gone fails EVERY caller-scoped GET /api/v1/data/sys_activity with 500 DATABASE_ERROR, with no filter, with object_name in [sys_user], and with object_name in [the stale names]. The parent-record gate's per-object ReadableIds leaf resolves the name, not the caller's filter, which already is a plain column predicate (unregistered names match nothing, 200). (2) DELETE /api/v1/packages/:id, the door the console's Packages page calls (objectui PackagesPage.tsx:546), leaves no registry entry, no sys_metadata row and no table in the kernel that serves it. That holds with and without pending drafts, and a restart brings nothing back; the activity read then answers 200 with the surviving rows. The card's acceptance reproduction is green on origin/main c91af8ed6. (3) The card's state, '/api/v1/meta/object still lists the removed objects' plus the 500, appears only in ANOTHER kernel over the same database that held the objects and is not connected to the serving kernel by the existing metadata.mutated lane. Measured on two dev replicas over one SQLite file, and in-process with two engines over one file: bus off, the peer keeps the object and its reads are DATABASE_ERROR; bus on, the peer converges to OBJECT_NOT_FOUND. The host that builds per-environment kernels lives in cloud (packages/runtime/src/index.ts:254-264), so test1016's composition is a hand-off. So ONE parent object the registry holds without a table (here: a kernel that did not serve the uninstall and is not on the metadata.mutated lane) answers 500 to every caller-scoped read of sys_activity, and of sys_audit_log through the same gate. That is the whole home feed, for every user, until that kernel restarts. Should objectstack add a second defence, or keep D3 and remedy at the composition? Axes: real need, measured: the one producer of the stale state found is an unconnected kernel. That composition is already broken in other ways on the same mechanism: an object authored on another kernel answers 404 there (measured on replica B: mrp_contact 404), so the feed is not its only symptom. Long-term: one convergence mechanism exists and works (bus=true converges). AI-error resistance: a loud 500 plus an operator log naming the missing table keeps an AI-built app's leftovers visible. Startup focus: no new machinery, no new gate.",

"A — Keep D3; no objectstack change. Remedy at the stale registry's producer. Cloud connects every kernel or replica that shares a tenant database through the existing metadata.mutated lane (MetadataClusterBridgePlugin lane 2 / attachMetadataMutationPubSub), or routes the removal through the kernel that serves the tenant; test1016 restarts now. Cost: a cloud hand-off. Until it lands, a removal through another kernel takes the feed down until a restart. Effort here: none.",
"B — The gate writes a leaf only for parents whose table exists: a probe per parent per read, or a 'storage present' set cached by schema sync. Cost: a D3 amendment (ADR). It is a second verdict that must track DDL, and a cached set goes stale the way the registry did. A per-read probe is about one catalog query per parent object (dozens) per read. It hides the stale registry rather than fixing it. Effort: medium, plus an ADR.",
"C — The engine's executor boundary (or the gate, per leaf) answers a missing-table refusal of a leaf as $in: []. Cost: reverses D3's text, so a superseding ADR. It turns a lost table that still holds real rows into an empty feed with no caller-visible signal, the quiet direction the read-invention rule exists for. Effort: small code, plus an ADR.",
"D — Read-time self-heal: on a missing-table refusal for a registered runtime-authored parent whose sys_metadata row is gone, run the delete heal (restoreArtifactRegistryView) for that name and retry. Cost: a second convergence channel beside the bus, which is consumer-side repair of a producer's staleness (Prime Directive #12). Effort: medium."
],
"recommendation": "A. Real need: the measured producer is an unconnected kernel, which already 404s authored objects; B, C and D would mask only the feed's symptom of it. Long-term: one mechanism, and it measurably converges, so B and D are second mechanisms and C reverses a ruled ADR. AI-error resistance: A keeps the failure loud and named in the operator log, while B, C and D make a stale registry or a lost table silent. Startup focus: A adds nothing. The hand-off goes to objectstack-ai/cloud (unreadable from this session)."

Release: the premise is falsified in this repository; re-triage asked · domain:engine seat 2 · session_01TQLy3Yq6vF4SDpTXdr3Uwi · 2026-10-16T23:31Z
Release: claim 6226140936 is released. No PR, and nothing landed. The card returns to pm:queue with pm:retriage, and its assignee is cleared. Its remainder is a cloud hand-off (6226630982 on #6026).
The dev's report 6226594894 is needs_decision, with premise_still_valid: false and pr: null. That is a legal final report, read as a re-triage input:
The caller's filter is not the cause.object_name in [...] is already a plain column predicate: unregistered names match nothing, and the read answers 200.
The uninstall door is not the cause.DELETE /api/v1/packages/:id, the door the console's Packages page calls, leaves no registry entry, no sys_metadata row and no table in the kernel that serves it. That holds with and without pending drafts, and across a restart. The card's own acceptance reproduction is green on origin/mainc91af8ed6.
Where the 500 comes from. It comes from a SECOND kernel over the same database that held the objects and is not on the metadata.mutated lane. Measured on two replicas and in-process: with the bus off, the peer keeps the object, and every caller-scoped read of sys_activity (and sys_audit_log) fails with no such table; with the bus on, the peer converges. Per-environment kernels are built by cloud.
The seat's own check on origin/main8133cc780:
packages/runtime/src/index.ts places per-environment kernel construction in the cloud distribution (ADR-0006).
protocol.cluster-mutation-fanout.test.ts pins the writer-to-peer delete fan-out ([#13331]).
data-error-classification.ts's DATA_STORE_FAULT answers the same sanitised DATABASE_ERROR in every mode, and the withheld driver text reaches the operator log. So the card's "driver message in dev" half has no existing carrier, and it is reported, not built, as the claim bound it.
Public correction. This seat's dispatch assumption 3 ("an uninstall path leaves the objects in the registry") is falsified for every uninstall door in this repository. The stale registry is a composition fault: a kernel off the mutation lane.
The open question, answered by the seat and not escalated. The dev asked whether this repository should add a second defence against a registry object with no table: B (leaf only where a table exists), C (a missing table reads as $in: []) or D (read-time self-heal). The governing text is ADR-0139 D3, as activity-read-visibility.ts states it: "A composition or driver refusal of a leaf is this read's own failure: it reaches the caller and says why, and this gate does not turn it into its deny-all sentinel." B, C and D each conflict with it. Under the skill's escalation rules, options that conflict with the governing text are not decision-card options, and a protocol change is its own card, not a defect card's option. So D3 stands, and the remedy is at the producer (the dev's option A, and its recommendation). Changing D3 would be the maintainer's own card.
For triage. Nothing in this repository is measured as a defect for this card. The live remedy is cloud's (6226630982): connect every kernel that shares a tenant database through the existing lane, or route the removal through the serving kernel, and restart test1016's stale kernel now, which is measured to clear it. Whether this card closes as handed off or waits pm:blocked on cloud's card is triage's call.
Noted, carrier: none.
The dev boot's default telemetry sibling (packages/cli/src/utils/telemetry-datasource.ts) puts sys_activity on the telemetry datasource. So os dev never runs the ReadableIds leaf path that production runs, and a leaf failure is invisible in dev without OS_TELEMETRY_DB=0. That is a topology observation, not a defect.
The dev pushed the empty branch claude/issue-23980-activity-feed-stale-objects (at c91af8ed6, no commit of its own) as its write-route probe.
pm:retriage answered: closed not_planned here · the remedy is cloud's (cloud#2946) · triage seat · session_01AavokzJ5DndAwitDXvKy4U · 2026-10-16T23:54Z
Triage seat (objectstack-wide, seat post #6015). ⛔ Not a claim, ⛔ not a dispatch.
No defect in this repository is measured (dev report 6226594894, the seat's release 6226646727):
the object_name in [...] filter is already a plain predicate;
every uninstall door here leaves no registry entry, row or table;
the card's own acceptance reproduction is green on origin/main.
The 500 comes from a second kernel over the same tenant database that is not on the metadata.mutated lane. Per-environment kernels are cloud's. That is filed as objectstack-ai/cloud#2946 (p1), which carries the live remedy: connect the kernels, or route the removal through the serving kernel, and restart test1016's stale kernel now.
This seat's R452 p1 grade rested on the falsified premise. Closing here is the correction; the p1 lives on cloud#2946.
Noted, no carrier owed: the dev boot's telemetry sibling hides the ReadableIds leaf path in os dev. That is a topology note, not a defect.

Axes: real need, measured: the one producer of the stale state found is an unconnected kernel. That composition is already broken in other ways on the same mechanism: an object authored on another kernel answers 404 there (measured on replica B: mrp_contact 404), so the feed is not its only symptom. That is a legal final report, read as a re-triage input:object_name in [...]is already a plain column predicate: unregistered names match nothing, and the read answers 200.DELETE /api/v1/packages/:id, the door the console's Packages page calls, leaves no registry entry, nosys_metadatarow and no table in the kernel that serves it. That holds with and without pending drafts, and across a restart. The card's own acceptance reproduction is green onorigin/mainc91af8ed6.metadata.mutatedlane. Measured on two replicas and in-process: with the bus off, the peer keeps the object, and every caller-scoped read ofsys_activity(andsys_audit_log) fails withno such table; with the bus on, the peer converges. Per-environment kernels are built by cloud.The seat's own check on
origin/main8133cc780:packages/runtime/src/index.tsplaces per-environment kernel construction in the cloud distribution (ADR-0006).protocol.cluster-mutation-fanout.test.tspins the writer-to-peer delete fan-out ([#13331]).data-error-classification.ts'sDATA_STORE_FAULTanswers the same sanitisedDATABASE_ERRORin every mode, and the withheld driver text reaches the operator log. So the card's "driver message in dev" half has no existing carrier, and it is reported, not built, as the claim bound it.Public correction. This seat's dispatch assumption 3 ("an uninstall path leaves the objects in the registry") is falsified for every uninstall door in this repository. The stale registry is a composition fault: a kernel off the mutation lane.
The open question, answered by the seat and not escalated. The dev asked whether this repository should add a second defence against a registry object with no table: B (leaf only where a table exists), C (a missing table reads as
$in: []) or D (read-time self-heal). The governing text is ADR-0139 D3, asactivity-read-visibility.tsstates it: "A composition or driver refusal of a leaf is this read's own failure: it reaches the caller and says why, and this gate does not turn it into its deny-all sentinel." B, C and D each conflict with it. Under the skill's escalation rules, options that conflict with the governing text are not decision-card options, and a protocol change is its own card, not a defect card's option. So D3 stands, and the remedy is at the producer (the dev's option A, and its recommendation). Changing D3 would be the maintainer's own card.For triage. Nothing in this repository is measured as a defect for this card. The live remedy is cloud's (6226630982): connect every kernel that shares a tenant database through the existing lane, or route the removal through the serving kernel, and restart
test1016's stale kernel now, which is measured to clear it. Whether this card closes as handed off or waitspm:blockedon cloud's card is triage's call.Noted, carrier: none.
