Repeated SelectList keys leave stale rows on screen. Index and FK names
repeat across tables, PostgreSQL overloads share a name, and rule
descriptions are free text.
The binary read .env through Bun (expanding $, cutting at #), and MSSQL
connected via master, which contained and non-VIEW-ANY-DATABASE logins
cannot. Connection errors name their reason and log the server's code;
global TUI shortcuts hold while a text field has input.
A file that runs past 10s now gets a server status report every 10s
(elapsed, pg_stat_progress_* / percent_complete / stage counters, lock
blockers) via file:progress, polled from one lazily checked-out side
connection held until the run ends. The run and change TUI screens
render it; Run Build and Run Exec cancel on a second Esc within 2s,
sending pg_cancel_backend / KILL QUERY through RunContext.signal.

Closes #98
Build, run, and change execution used one or two tracking queries per
file to decide skip-or-run, and wrote a row for every skipped file. One
prefetch per batch now feeds a pure decision function; skipped files
write nothing, and a build where nothing changed writes no operation.

Also fixes run.file(path, { preview: true }) executing the SQL, and
checkFilesStatus matching absolute paths against relative rows, which
kept the TUI re-run prompt from ever firing.
