Current Behavior
This issue affects various components within a form when teh form is placed within a modal. Faran has already confirmed this bug and he instructed me to raise this bug.

The behaviour described can be seen in the attacehd app file.
modal form default field.json
The current workaround is to set the default value of each control individually, but this does not work for the autocomplete widget.
Expected Behavior
I expect the form defaults to work when the form is placed in a modal.
