fix(web): two-factor fixes: no remount on token refresh, factor carries across a site switch (web 0.62.1) - #613
1. Token refreshes no longer remount signed-in pages.
RequireAuth (and MfaGate) keyed their "owes a second factor" check by the access token. Every token change, the hourly refresh or passing the authenticator code, reset it to unknown and replaced the page with "Loading...".
Visible effects since 0.62.0: turning on two-factor lost the recovery codes it had just made, and signed-in pages lost their state about once an hour.
The check is now keyed by the user and keeps its last answer while it checks again.
2. Two-factor carries across a site switch (migration 00088).
A switch between webhooks.cc and mailhooks.cc signs in with a fresh magic-link session. GoTrue rates it aal1 and has no way to raise it, so the code was asked again on every switch.
When the session the switch started from had passed the second factor, the new session is listed in mfa_trusted_sessions. That is checked when the grant is made, and again at issue against this browser's cookie session.
Every aal2 check accepts a listed session: session routes, ops, device approval, recovery codes, and session_mfa_ok() for RLS.
The browser's code prompt asks /api/account/mfa/status before showing.
The row references auth.sessions with cascade, so signing out ends it. A password-only session never gets listed.
Deploy
Apply 00088_mfa_site_switch.sql before the web release. Without the table the new code treats nothing as carried, so nothing breaks; the code is simply asked as before.
Tests
Browser: enrolled an authenticator through the real Security page. All ten codes stay on screen and no "Loading..." appears.
Integration: a password-only session on an account with an authenticator is refused before trust and accepted after, by both the app and RLS. Signing out deletes the trust row.
Unit suite: 1,460 tests pass.
Not testable locally: the full switch round trip. Dev runs one site, so it is covered by its parts.
RequireAuth keyed its "owes a second factor" check by the access token,
so every token change (the hourly refresh, or passing the authenticator
code) reset it to unknown and showed "Loading..." in place of the page,
unmounting it. Turning on two-factor sign-in lost the recovery codes it
had just made that way. The check is now keyed by the user and keeps its
last answer while it checks again.

Walkthrough
The second-factor hook now uses the session user ID and access token to check assurance level. The web package version and changelog are updated to 0.62.1.
Changes: Web authentication and release
Layer / File(s)
Summary
Second-factor check inputs and state apps/web/components/auth/mfa-challenge.tsx, apps/web/components/auth/require-auth.tsx
The hook accepts a user ID and token. It reruns the check when either changes, retains the result for the same user during token changes, and returns null until that user has a result. Both callers pass the session user ID and access token.
Web release metadata apps/web/lib/changelog.ts, apps/web/package.json
The package version and changelog are updated to 0.62.1. The changelog entry describes recovery-code display and background session renewal.
The change preserves page state during token refresh, but a session newly requiring MFA can briefly retain protected UI before showing its challenge. Server checks limit the impact; this is mergeable with awareness of that bounded gap.

Explanation
The title names the two-factor fix, the no-remount-on-token-refresh behavior, and the factor carrying across a site switch, which matches the changes to useNeedsSecondFactor and RequireAuthInner. It is long and lists two fixes, so it is broader than one main change, but it is related to the changeset and is not vague.

Switching between webhooks.cc and mailhooks.cc signs in with a fresh
magic-link session, which GoTrue rates aal1 and has no way to raise, so an
account with an authenticator had to enter its code on every switch. When
the session the switch started from had passed the second factor (checked
when the grant is made and again when the ticket is issued), finish lists
the new session in mfa_trusted_sessions (migration 00088), and every aal2
check accepts it: the session routes, ops, device approval, recovery codes,
and session_mfa_ok() for RLS. The browser's code prompt asks the server
before it shows. A row goes with its session, so signing out ends it.

Keep the second-factor gate closed while a new token is checked.
41-41: Minor · Functional Correctness · Poor tradeoff
Keep the second-factor gate closed while a new token is checked.
When the session token changes to one that needs a second factor, the retained false answer lets protected children render until the new check resolves. RequireAuthInner and MfaGate both render children on that stale answer, so the challenge appears late. The API still rejects AAL1 sessions. Consider hiding children until the answer for the current token is known, without unmounting them.

Update the MFA state handling around the userId check so a retained false answer
is not treated as resolved for a changed session token. Keep protected children
mounted, but hide them until the second-factor result for the current token is
known; preserve the existing rendering behavior once that result resolves.

Key verified to the user who passed the check.
verified is a plain boolean. If another tab signs in as a second account while this instance stays mounted, the second account's protected children render without its challenge. Track the verified user ID instead.

Replace the account-agnostic verified boolean in the authentication guard with
the ID of the user who completed the challenge. In the guard’s check around
useNeedsSecondFactor, require verification for the current session?.user.id and
record that ID when MfaChallenge succeeds, so verification does not carry over
when the signed-in user changes.
Key the MfaGate verification state to the signed-in user.

Key the MfaGate verification state to the signed-in user.
verified belongs to the component instance. It does not change when the signed-in user changes, so the gate can keep one account's verification while another account is signed in. A sign-in from another tab can produce this. For example, a password sign-in for account B that was submitted before this tab received account A's SIGNED_IN can complete afterward. Its SIGNED_IN event then updates this tab's session to B.
Account A passes the challenge in this tab, so verified becomes true.
The session changes to account B. needs is recomputed for B and returns null until its check completes.
When the check returns needs: true for B, verified is still true. The children render without B's challenge.

Update MfaGate’s verification state to be associated with the current session
user ID, and consider verification valid only when that ID matches; record the
current user ID when verification succeeds. Preserve the existing needs-check
behavior so a new user remains gated when useNeedsSecondFactor reports that a
second factor is required.

RequireAuth and MfaGate kept a plain "verified" flag, so if another
account signed in from another tab while the page stayed mounted, it
could inherit the first account's pass. The flag is now the user id that
passed the code.

RequireAuth and MfaGate, key verification to the user: fixed. Both now store the id of the user who passed the code, so an account signed in from another tab has to pass its own.
Keep the gate closed while a new token is checked: left as is, on purpose. Hiding the page on every token change is the remount problem this PR removes: the hourly refresh would blank the page each time. For the same user, a token that starts owing a factor while the page is open would have to come from another tab enrolling an authenticator. That upgrades the enrolling session to aal2 and leaves this one as it was. In that window the server refuses the session anyway (owesSecondFactor in api-auth, session_mfa_ok() in RLS), so the page only renders shells with nothing to load until the check resolves and the prompt appears.

From a review of the site-switch change:
- A trigger on auth.mfa_factors drops an account's carried sessions when a
factor is removed or newly verified (recovery code, staff reset,
unenrolling, a new phone), and removeTwoFactor() drops them too; a
carried session also counts only while the account has a factor.
- Recording the trust is ON CONFLICT DO NOTHING, which needs only the
INSERT grant the migration gives.
- The grant asks with the full user, factors included.
- The Security page asks for the code once before turning two-factor off
in a carried session, which GoTrue still rates aal1.
