Summary

Adds plugin/.claude-plugin/icon.png, a copy of the app's 512×512 icon (icons/icons/png/512x512.png).

The Claude directory's validation of the plugin from #1337 passes with one warning, "No icon", and it reads the icon only once, at the plugin's first submission. So it has to be on main before that submission.
