Closing this in favour of #45.

This PR bumps send to 1.1.0, which only exists on the express 5 line, so it
also drags express to a major. #45 stays on express 4 and bumps it to 4.22.2,
which brings send 0.19.0. That carries the fix this PR was chasing, without
the major.

After #45, npm audit reports no vulnerabilities and the repo has no open
send advisory.

express here is a dev dependency. It drives the local demo harness in
index.ts and never reaches the published tarball, which is the 8 files under
dist/.
