[BUG] Continuous re-delegation

Describe the bug

I'm having an issue where I start in orchestrator mode and every spawned subtask thinks it's still in orchestrator mode and re-delegates. I've seen this go 30+ deep.

To Reproduce
Steps to reproduce the behavior:

1. Go to '...'
2. Click on '....'
3. Scroll down to '....'
4. See error

Expected behavior

Roo to recognize the current mode and act accordingly

If I stop it before it's able to spawn the next sub-task, it recognizes that it's actually in code mode and will continue properly

i'm using deepseek and it keep prompts me this Zoo is having trouble... This may indicate a failure in the model's thought process or inability to use a tool properly, which can be mitigated with some user guidance (e.g. "Try breaking down the task into smaller steps").Details
