fix(runner): retain synchronous provider results

🤔 Background

Related #1377, #1385

An opted-out provider file could print a failure and still exit successfully in a parallel run. The cancellation regression also mistook an exited, unreaped body for running cleanup.

💡 Changes

• Give every dispatched test its own result ordinal, covering summaries, reports, hooks, retries and repeats.
• Check cancellation process state while retaining the live-body failure and existing time limits.
• Give skipped macOS and Windows acceptance jobs static names, with shard labels in test steps.
• Verify source and standalone runners on Bash 3.0 and macOS Bash 3.2, a controlled exited-body reproduction, and the full local gate.
