Implementation Plan: Implement directly from issue
This PR contains the implementation plan for #17.
The plan is open as a draft for author review and sign-off before development begins.
Once approved, this branch will be closed and development will proceed on a feature branch.
Plan summary
The plan adds a lightweight express path that routes eligible issues directly from triage to implementation, skipping intermediate planning phases while producing a minimal traceability artifact. The express path operates alongside the existing feature and bugfix flows without modification. Work is organized into 7 phases: triage-issue skill extension, express flow configuration, decision artifact creation, telemetry/logging, alerting/dry-run tests, verification/hardening, and documentation/rollout.
