Feat/custom prompts runtime
Anthropic rejects assistant messages where text content blocks appear after tool_use blocks. DCP nudge and message-id injections were using push() which appended text after existing tool parts, causing 'tool_use ids found without tool_result blocks' API errors. Use splice() to insert before the first tool part instead. Remove now-unused model-specific synthetic tool part fallback (rejectsTextParts, createSyntheticToolPart, isGeminiModel) and modelId plumbing.
