Add DBM blurb and setup alert to ClickHouse README

What does this PR do?

Adds a Database Monitoring (DBM) blurb to the ClickHouse integration README and an alert div in the Setup section that redirects users looking for DBM to the dedicated setup page.

Motivation

The ClickHouse DBM feature is in preview. The standard integration README should surface this for users who land on it while looking for query-level observability, and direct them to the correct docs.
