Plan mode not switching when AI asks to do changes

Describe the bug

When you are in plan mode and the AI has shown you the changes to be made it will ask you if it should go ahead and if you say yes it will still show Plan Mode but going forward it does not ask. Appears it turned off plan mode but did not update the UI.

Sometimes it does say switch out of plan mode and then tell me to go ahead with the changes. I have not tried to say go ahead without switching to see if it will say still in plan mode

Steps to reproduce the behavior

Place in plan mode
Ask AI to make a plan to change code
When it asks to implement it and does not say switch from plan mode, tell it to go ahead
Once that happens the UI still shows Plan Mode but it no longer asks to make changes

Expected behavior

When in plan mode it never makes changes and if you tell it to then it confirms you want to exit plan mode and updates the UI.

Was in Plan Mode and asked this of the AI , now version 0.0.402

● You're right - I apologize. I made a judgment call that the task was too trivial for a plan (single file, one line), but I should have respected plan mode as you explicitly requested it.

I've had many problems with plan mode. It seems like it depends on the model you pick whether it will change code in Plan mode. For example, the Anthropic models seem to obey plan mode when OpenAI models seem to not care about it as much.

I am using Opus 4.5 and after several days usage it appears that the AI is not checking if the status is Plan Mode every time , seems to work on the first one , then is hit or miss after that.

Sometimes it will say Switch out of plan mode and then tell me to go ahead. but not all the time

We have hardened this area a lot in version 1.0.74. I am closing this issue because I believe that the things reported here are fixed now. If you think otherwise, please reopen the issue with details about the problems you encounter.
