Steps to reproduce

1. I send the prompt.
2. It generates an error.
3. I change the model.
4. It generates another error.
5. I close the OpenCode Desktop window.
6. I open the OpenCode Desktop window.
7. The error persists.
8. I open OpenCode Web from Debian using WSL on Windows
9. same error
