At Salesforce, internal tools can help but the customers need to have this information on their own

1. Record Lock Contention
When a parent record has 10K+ children, any DML operation (insert, update, delete) on a child record requires a lock on the parent record
2. Sharing Recalculation Performance
If the parent record's ownership or sharing changes, Salesforce must recalculate sharing for all 10K+ child records
This causes system delays org-wide
3. Account Data Skew (Most Common)
The most frequent form — one Account with tens of thousands of Contacts, Opportunities, Cases etc.
Severely impacts page load times for the parent Account record
Related lists become slow.
4. Query & Report Performance
Queries filtering on the skewed parent return massive result sets
Reports aggregating child records under the skewed parent are slow or time out
