Frequent 403 Forbidden errors during chat/file operations

Description:
Getting frequent 403 Forbidden errors during chat or file operations. When trying /compress to recover, it fails with a different error. So basically stuck and have to start a fresh session.

Errors:

First error (during normal usage):

Error: 403 {"detail":"Forbidden","status":403,"title":"Forbidden","requestId":"sin1::n97s8-1766288739206-569e9ddda723"}

Second error (when running /compress):

Error: Failed to compact and start a new session

Environment:

• OS: Windows 11
• Shell: PowerShell 5/7
• Model: Opus 4.5 high reasoning
